Problems people have learned to work around. Problems that live between teams, systems, or institutions—where responsibility is divided and the whole outcome belongs to no one.
The workaround may look small. Over time, it becomes the process. Cost, delay, uncertainty, and lost judgment accumulate around it.
That is often the signal: not that one thing is broken, but that the underlying system is ready to be reconsidered.
